If the form has never been completed by a member, you can delete it. From the administration panel, click on "View all". If the form can be deleted, you will see a remove link next to it.
If the form has been completed, it cannot be deleted. If you no longer want to use that form, change its name to something like "OLD - DO NOT USE"
